Your First Visit

We want your first visit to feel comfortable, unhurried, and genuinely helpful — and we want it to give you real answers about your condition, your options, and what comes next. From the moment you arrive, our team is focused on one thing: making sure you leave with the clarity and confidence to move forward.

Before Your Appointment

To help your visit go as smoothly as possible, we encourage you to complete your new patient paperwork in advance through our patient portal using your first name, last name, date of birth, and email address. Click here to access the portal.

If you prefer not to use the portal, paper forms are available upon request and can be completed in the office or provided to you in advance. If you are unable to complete forms independently, our team is available to assist you in a manner that respects your privacy and confidentiality.

We offer reasonable accommodations and alternative formats for patients with disabilities, including assistance with forms and other modifications as needed. To request an accommodation, please contact our office at (937) 907-9009 or [email protected]. Advance notice is appreciated but not required.

At Your Appointment

After checking in, we will take a few photographs and perform testing. These steps help us fully understand your concerns and make sure we have everything we need before you meet with the doctor. It’s a quick but important part of the process, and our staff will walk you through everything as it happens so you always know what to expect.

From there, you’ll have dedicated one-on-one time with one of our board-certified physicians. This is your opportunity to share your concerns openly, ask any questions you’ve been holding onto, and hear honest recommendations tailored specifically to you. We believe every patient deserves a thorough, thoughtful consultation — not a rushed overview — and we take the time to make sure you fully understand your options before any decisions are made.

To wrap up your visit, our surgery scheduler will sit down with you to review available procedure dates and locations, and walk you through everything you’ll need to do to feel fully prepared. From pre-procedure instructions to what to expect on the day of your surgery, we make sure no detail is left uncovered. We believe an informed patient is a confident one, and we’re here to make sure you leave your first visit feeling heard, supported, and ready for what comes next.
